## Changes:
- FIXED: `_D601` → `_[D]001` CCC-ID format — §3 rewritten with explicit formula (underscore + day digit + "001") and ALL 7 day examples shown. Resolves `_2601→_2001` (W31 D2) and `_3601→_3001` (W31 D3) ambiguity errors. IMMUTABLE #107 + L-431.5 🟡 PROPOSED.
- FIXED: Algorithm updated — COUNTER resets to 001 (not 601), BASE = {D}001. Subsequent increments: +1.
- CORRECTED: DRIFT-001 — v4.31.2-r1 violated #128 (~32% content loss). v4.31.2-r2 restores ALL 50,718+ characters, 8,399+ words, 888+ lines from v4.31.1-r1 baseline. Full content preserved.
- NEW: §18 Fix Documentation — Full `_D601`→`_[D]001` root cause analysis, 2-incident table, replacement formula, forever-learned declaration.
- NEW: §4.8 #LeanRAG8 Awareness enhanced with Source of Truth URLs for all 8 indexed docs (raw.githubusercontent.com)
