Rename the book reward bucket from Cloud to Book & Library in the guide and form pack

Browse Source 
The book and library tiers ($19 Early Bird eBook, $25 eBook plus updates,
$47 eBook plus 12 months of AiBook.fun, $55 signed paperback, $85 Collector's
Bundle) were grouped under "Cloud". WeOwn Cloud is a separate product, so the
bucket is now "Book & Library" everywhere. Sentences that called Cloud the
do-it-yourself path now describe the Book & Library tiers plainly. Prices,
dates and other content are unchanged. The .docx is regenerated from the
fixed Markdown with v2/build_docx.py (same page map, 36 pages).
